إنّ تكوين الجنون بوصفه مرضًا عقليًا في نهاية القرن الثامن عشر يشهد على حوار قد انقطع، ويفترض الانفصال قد تمّ من قبل، ويلقي في النسيان كل تلك الكلمات المتلعثمة الناقصة التي لا قواعد ثابتة لتركيبها، والتي كان يجري فيها التبادل بين الجنون والعقل. إنّ لغة الطب النفسي، وهي مونولوج العقل عن الجنون، لم تُقم إلا على أساس هذا الصمت.
